COVID-19 Rapid Testing Student Consent Form
Pearland ISD is adding a voluntary K-12 COVID-19 testing program for students. This program uses Abbott Laboratories BinaxNOW tests provided by the federal government. We will only test with your consent. In addition to written consent, you will receive a phone call before a test is administered to your child. You may provide consent for the district to administer this test on your child or yourself (if student age 18 or older), by completing the Consent Form below. The form is also available through Skyward Family Access.

My number one goal is to keep your students safe and healthy so that they can learn better in their school environment. There are a few ways you can assist us with this goal.
Illnesses: More than ever, it is important that you keep your student home when they are ill. Please follow PISD policy regarding symptoms of COVID-19. The district is asking that you screen your students prior to them leaving for school daily, and do not send them if they are experiencing symptoms/are ill. Please notify us of any positive COVID test results or contacts.
Criteria for Return to School
Conditions for return to school after an individual shows ANY symptom in a way that is unusual for them:
- 10 days have passed since symptoms first appeared; AND
- 24 hours with no fever; AND
- Symptoms improved
- An acute infection test that comes back negative for COVID-19
- A doctor’s note indicating an alternate diagnosis

Medications: Please give all medications at home when possible. If your student needs to take a medication during the school day, it must be in the original packaging. It will need to be left in the clinic with the nurse. For medications that will be given for 10 days or fewer, we will need a note or form with a parent signature. Anything that will be given for longer, even over-the-counter medications, must have a doctor's authorization form as well. These forms are available at the link to the School Health Services page.
Immunizations: Students in public schools must meet the Texas Minimum State Vaccine Requirements, even if they are remote learning. We will notify you of any vaccines that are due during the school year or over the summer. Please keep your child's shots up to date and provide us with copies of records/proof of immunization.
