1095-C Forms: Year 2024
Health Insurance Coverage
New legislation on optional ACA reporting relief states that ALEs are no longer required to send 1095-B and 1095-C forms to covered individuals and full-time employees unless a form is requested.
The legislation provides that reporting entities must notify individuals of their right to request a copy of the statement “at such time and in such manner as the [IRS] may provide” to take advantage of the alternative furnishing method. These requirements are now set forth in the IRS Notice 2025-15 attached, which also applies to 2024 calendar year reporting due in early 2025.

1.)Submit your request to Benefits Team at one of the following:

- View/download via Employee Self Service (ESS) online.
2.)Within the request, you will need to provide the following information:
- Employee ID #,
